I would like to express my thoughts on the care of Mom during her stay with you. What struck me first about your facility was the cleanliness and the lack of noise. Both wonderfully welcome. A great facility is nothing without caring kind people to both the patient and relatives. I would like to recognize the professionals who took care of mom. Their names are; Ross,Grace,Corrina, Kim, and numerous volunteers. All professional, kind, caring folks. You have to have a special wonderful personality to work in this environment.I think anyone who has attended school in the medical field is trained to save lives. This aspect of this business of dying equally hard for someone working this part of the medicine and to do it with kindness and respect. Your care is exceptional. You all have excellent folks working for you.

My brother recently transferred his hospice home care to transitions. “If I could give a 10 star rating, I would .” Each member on his Homecare team has been nothing but exceptionally professional. Your compassion, sensitivity in clinical ability of beyond remarkable. Each physician , clinician, the social worker and chaplain have been remarkable. It’s amazing what medication has been given to allow my brother that have a “0” pain level. Transitions is a treasure. If your loved one needs hospice care, please do not wait at the last minute. Be proactive so that quality of life is achieved. I only regret is that I wish I would have chosen transitions to begin with. Each of you are angels!

This review is specifically about Joshua McCullough. He has been an amazing contact point, guiding my family through this difficult process. If there were ever a person who deserves a commendation, it is him. I was so impressed and honestly touched by how much care he took with us.

I cannot express our family's gratitude for the care and consideration that was given to our father while staying at this facility. All of the employees that interacted with us were courteous, caring, and professional at all times . We felt that each employee genuinely cared about our father and our well-being for the short time we were there.

The open door policy allows me to visit my father whenever he needs me and whenever I need him!

First, my father in law was at Transitions for hospice care. I was so impressed. I loved this place. Then when my dad was dying, I wanted him to go to Transitions. The hospital social worker had Transitions call me. I was told he would have to have hospice at home. I wanted him to go to their facilties. She said, "he's not in pain" . What He was in pain. He was being given pain and sedation meds through an IV. The lady said they can bring a bed to his house the next day. I told her she was rushing me. I later learned that they take patients who are on IV end of life medications. I don't know why they said my dad didn't qualify. He passed the next day after the discussion that he wasn't in pain. He passed in Rex hospital. With wonderful care.

I visited a beautiful friend at this location today. I was so impressed with the calming atmosphere presented. This place is decorated with colorful art and kind reminders of home. The rooms have large windows that provide a glimpse of nature and greenery. I appreciate the kindness and attentiveness of the staff.
