Building looks nice. But they kept my husband hostage until I threatened them with an attorney. I had to drive from Phoenix to literly take him from the facility. They didn't want to give up the $500 a day the VA paid them. Also he was fed very small portions . He went from 126 to 116. He look skeletal. These people would never return my calls. I am his wife. Who are they to decide what I should know. He is in a 100 percent better place in Phoenix. They give him huge portions of food. Has already put on a couple of pounds. No I don't recommend this facility

They took fabulous care of my mom and I would recommend this place. They were always welcoming to me when I came to visit and they all loved my mom so much. She passed away peacefully and I can’t brag on the staff enough for doing an amazing job and always being so kind to me throughout the entire process.

I beseech you, beg you, and plead.... DO NOT SEND YOUR LOVED ONE HERE. A friend of ours was admitted in December and I literally saw him go from having the desire to live to fading away. Since we weren't in any authority to advocate legally, We could only encourage him and try to make contact with his family. He was wanting to be transferred to another facility but it didn't happen quick enough. He had a fall getting out of bed, then suddenly was "sleeping" all day and stopped eating or drinking fluids. I honestly believe he was kept sedated to stay in bed and not be a fall risk. Sadly within about 2 weeks after his fall he's gone and this facility continues on. Although the staff tried to show support and concern in his last days, I knew they were only trying to look good for the visitors. During previous visits and attempts to get the attention our friend needed, it was obvious the care for patients was minimal. This facility can't be the only option... Make every effort to advocate for your loved one... You won't regret it.

Mom needed more help as she aged and finally the day came when I could no longer keep Mom independent. After finding Rim Country Rehab in Payson, all my worries about Mom's care were alleviated by the team of caring professionals that went above and beyond palliative care. In the past I gave Rim country a 5 star ⭐ and all of the staff are great and take very good care of my mom, however I have a problem with Dr. Michaels because he will not write orders for my mother to have her urine checked twice monthly due to her frequent UTI'S. Emergency room visits and hospital admissions could be eliminated or less often. In the past month she has been in the hospital with severe UTI she was septic and very sick and the last admission she had to be flown from Banner hospital to Honor hospital in Scottsdale Arizona unresponsive and was there for a week all due to UTI'S I feel that my mother is being neglected by Dr. Michaels because he's the one who has to write the orders to check her urine and put her on antibiotics if needed the nurses are doing the best they can but they cannot run a uac&s without the doctors orders

I was in a bad car accident and had to have surgery. After surgery, I was transferred to a rehab facility in the valley that I was not at all happy with. After a week there, a friend recommended Rim County Health and Rehabilitation. The difference was NIGHT AND DAY! I have been here for about 3 weeks and could not be happier. It is CLEAN and all of the staff, from the Director, to the Nurses and CNA’s and Physical Therapy, to Housekeeping are all WONDERFUL. They are knowledgeable, attentive, kind and encouraging. I never feel like I’m putting anyone out, and I always feel like I’m their first priority. The food is great and they have a variety of snacks that I can have at any time during the day or night.
This is a place that I would feel completely comfortable sending my own mom or dad, should the need ever arise. I know they’d be in wonderful hands and well taken care of. Rim County Health has gone above and beyond to treat ME, not just my injuries. I’ve never felt scared or alone here, either. I constantly have the staff checking in on me to see if I need anything, if I’m comfortable, hungry, etc. That’s very important when you have to be away from your family.
I rarely write reviews, but I Highly recommend this place for your loved ones. You will not need to worry while they are here. We are WELL
taken of. Thank you so much to all of the staff! I am going to miss you all when I go home next week.

I am currently going through rehab and the staff is just wonderful. Lynn, Rebecca, Jeff, Peyton, Alan, and the entire staff is friendly, respectful, knowledgeable, and very thorough. Everyone is so nice and easy to get along with! They take such good care of everyone here. I was a patient here at the beginning of the year and I was treated superbly, and professionally. My stay was absolutely wonderful. I cannot say enough of the many positive virtues and standards that I experienced here! I am currently an outpatient now but I would highly recommend this establishment for those who need special care; especially the elderly. The food is great and there's plenty of activities to participate in. The rooms are kept very clean and we'll maintained. They also have special equipment available depending on your situation. Give this place a call and ask them plenty of questions regarding your specific needs. They also have a brochure for your loved ones that explains and shows how their entire facility operates. The nursing staff is awesome! They are very attentive to all your needs and they will ensure that all your needs are met. This is a top notch first class place. I'm so proud of them! Keep up the great work guys!

Do not send your loved ones to this facility. I Experienced the worst customer service when trying to contact my father who was on hospice. I called my father every day for a week after I was off work at 5pm and every time I called the “Lookout” facility the woman was rude and not welcoming. On 02/10/2022 I called every 30 minutes to try and speak with my father and no one answered the phone from 5-8PM. I even tried different extensions and was transferred to the same voicemail. The next morning I was informed that my father had passed away. The morning of 2/11/2022. If it weren’t for their negligence I could have spoken to my father one last time.
